    name of player's held item is "&aSpecial Paper"
    execute console command "yourcommandtogiverank"

i don't understand the first part of what you wrote, player will gets after click

on right click:
    type of player's held item is paper
    name of player's held item is "&aSpecial Paper"
    execute console command "yourcommandtogiverank"
    remove 1 of paper named "&aSpecial Paper" from player

if you havent thats the first thing you should try
- set a variable to an %item% named %string% with lore %{_listOfStrings}%
