What I want:
Crawling without the need of trapdoors.

To crawl, look straight down and sneak.
To stand up, press space.

It's not possible.
From what I have been told, the crawling state is client side only, and the server doesn't recognize it.

I've seen this done by spawning a shulker over the player.
