    You will need to use the skript-advancements addon for skript because I couldn't find a event for getting an achievement in vanilla on advancement: # will need the skript-advancements addon add 1 to player's max health

    most likely you need to use .5 behind the location of a block but to be completely sure just debug by checking the location of every sign you right click and set the location to that like this: on right click on sign: broadcast event-location
